We have a rare and exciting role for an RN in our Case Management department. You get to work as a team with a Social Worker to provide supportive client centered HIV/AIDS case management services (AIDS Medi-Cal Waiver Program) in Sacramento and Yolo counties.
Ideal for RNs seeking
- A patient-centered role with minimal physical demands (mostly vitals, questions, and charting).
- Ideal daytime hours (Monday to Friday, days), no shift work, no rush from patient to patient (our patients are clients).
- A friendly office environment, visiting client homes with a social worker.
Requirements
- CA Licensed RN, 2 years minimum. Minimum 1 year in a community, public health, or in-home nursing role required.
- Able to work in office environment initially and eventually go to client homes. (Remote work is not an option.)
- Thrive on helping a diverse group of clients in their home setting; good people skills a must.
- Be able to help clients remain connected to appropriate medical/mental health/AOD/community services with a goal of stabilizing their health, meeting their needs, and avoiding higher levels of care.
- Experience providing similar or related services and/or working with this population preferred, but we can train the right candidate.
