Rx HealthCare Services is able to assist you if you're recovering from operative surgery, physical trauma, or any form of hospital treatment that requires monitoring upon being discharged to your home. Our specialized home health team works closely with other service providers to personalize your care and we ensure that all of your needs are met.

Referrals
Referrals for home health are made by physicians, hospital discharge planners, and other medical providers. You then have the option to choose your preferred home health agency to provide care in your home or place of residence. A nurse or therapist will make a thorough assessment of your health care needs and coordinate a plan of care with your physician.

Rx HealthCare Services (Rx Staffing and Home Care, Inc.) is a Medicare-certified home health agency at 4640 Marconi Avenue, Suite 8, Sacramento, CA 95821. Call (916) 485-8200. A nurse is on call 24 hours a day, 7 days a week at the same number.

Service limitations
- We serve adults only; we do not serve pediatric patients.
- Speech therapy depends on therapist availability. It is not currently available in Galt, Lincoln, Loomis, Yuba City, Auburn, Marysville or Sutter.
- Occupational therapy is not currently available in Auburn, Marysville or Sutter.
- We do not offer pharmacy, laboratory or outpatient therapy services.
- Visit frequency and duration follow the physician's plan of care. Acceptance depends on our current staffing and caseload in the patient's area and discipline. If we cannot accept a referral, we tell the referral source promptly.
How to refer
Call (916) 485-8200. Please include the patient's diagnoses, physician orders for the disciplines needed, expected visit frequency and duration, homebound status, any specialty needs (for example wound, ostomy or tube care), and recent hospital or facility information if there is any.
