Case Management

Rx HealthCare Services AIDS Project is a state-funded program that allows people living with symptomatic HIV or AIDS to receive care and remain in the comfort of their own home. Our great team of case managers work with every client to monitor their needs and connect them with many community resources. This service is free and we service both Sacramento and Yolo Counties.

Skilled Nurses

Our Case Managers work in coordination with our Home Health Department to provide skilled nursing in your home. Skilled Nursing is provided by RNs and LVNs whose main function is to assess and evaluate your nursing needs, and to perform rehabilitative and preventive procedures that are instructed by your physician for your own safety.

Short-Term Psychotherapy

Short-term psychotherapy is provided by our qualified professionals to assist clients in the psychological adjustment of living with HIV/AIDS.

Attendant Care Services

Attendant care services are provided by Certified Home Health Aides who perform hands-on care, which may include preparing of nutritious meals, taking vital signs, reporting changes in your condition, assisting you with bathing, dressing, walking, eating, and performance of other light housekeeping activities.

Homemaker Services

Homemaker services consist of general household activities which may include sweeping, vacuuming, dusting, washing dishes, cleaning the bathroom, taking out the garbage, changing linens, laundry, shopping and short errands. We provide these services to ensure you can live safely and independently.

Non-Emergency Medical Transportation

Non-emergency medical transportation is the provision of transportation vouchers for public transit or gas vouchers to obtain health and/or social services such as counseling, support groups, and methadone treatment.

Other services we provide include:

  • Assistance in locating local physicians and HIV clinics
  • Assistance in locating confidential and anonymous HIV testing sites
  • Assistance in obtaining medical equipment and supplies
  • Provision of resources for legal services

Eligibility Requirements

  • Have a diagnosis of disabling HIV or AIDS
  • Display HIV/AIDS-related symptoms
  • Be under the care of a physician
  • Reside in Sacramento or Yolo County
