Palliative Nursing Care at Home

At Rx HealthCare Services our Palliative Care Program model:

⦁  Provides symptomatic supportive care to patients in between doctor visits
⦁  Helps patients navigate complex medical conditions in their home setting
⦁  Provides connections with community resources
⦁  Offers pain and medication management with RN oversight
⦁  Provides a full professional interdisciplinary team for the patient
⦁  Decreases potential for ER visits and hospitalization
⦁  Is covered by Medicare and most insurances

Palliative care is available for:

⦁  Chronically ill patients who may or may not have a terminal prognosis
⦁  Patients who have a deteriorating medical condition requiring symptom management
⦁  Patients who require a taxing effort to leave their home setting
⦁  Patients who are currently seeking concurrent curative or rehabilitative treatments

Q: How does palliative care differ from hospice care?

A: Palliative care may be provided while you are actively seeking curative care while hospice care may not, and palliative care is not limited to people with a medical prognosis for life expectancy of six months or less.

Q: What are eligibility requirements of palliative care vs. hospice care?

A: Palliative care is begun at the discretion of the physician and patient at any time, at any stage of illness, terminal or not.

Hospice eligibility requires that two physicians certify that the patient has less than six months to live, if the disease follows its usual course.
