Traditionally home care is considered an area that only serves the elderly, which is utilised by those who don’t require the care provided by an aged care facility. These days, home care refers to the care of athletes, young people, and adults recovering from injury or illness, as well as the care of elderly.
Health care services at Home
Rx HealthCare services aim to provide people recovering from operative surgery, physical trauma, or any form of hospital treatment requiring post-treatment monitoring with the peace of mind of recovering in the comfort of their own home.
Referrals for home health are made by physicians, hospital discharge planners, and other medical providers. A nurse or therapist will then assess your health care needs and work with you to determine a thorough plan. We tailor your care to suit specific needs and circumstances, giving you the freedom to carry out areas of your daily routine that may have been limited under hospital care.
Rx HealthCare Services are committed to providing temporary patients with the same level of care as our long-term patients. We ensure each patient receives individual care during recovery and a personalized rehabilitation program in the later stages of recovery.
Hospice
Rx HealthCare Services pride ourselves on providing exceptional service and care for individuals with a life expectancy of six months or less. Improving and emphasizing quality of life for our hospice patients is our end goal and by using methods of pain relief symptom management, we allow patients the freedom and comfort to live the remainder of their life to the fullest.
Rx patients will be cared for by a team of doctors, nurses, therapists, social workers, clergy aides, dieticians, and volunteers, all working to provide the highest level of palliative care possible. We work to control pain and provide morale support in hard times.
Hospice patients and their families have access to 24/7 support. With hospice patients being at the highest risk of medical emergency, our support team are always just a phone call away.
Caregiving
For those who want to remain self-sufficient but are in need of assistance for a number of day-to-day activities, caregiving can be the perfect solution. Caregiving provides the patient with the freedom of living at home and not sacrificing their regular routine, whilst allowing our team to assist with daily tasks, whether it be for a few hours, a couple of days, or long-term. Our services vary and are based on the individual needs of each patient. These include:
- Hourly or live-in care
- Bathing
- Dressing
- Grooming/personal care
- Transferring/mobility assistance
- Safety monitoring/supervision

Case management
Rx HealthCare Services case management allows patients living with symptomatic HIV or AIDS to receive care whist remaining in the comfort of their own home. This project is a free, state-funded program that we run in both Sacramento and Yolo Counties.
Our RNs and LVNs assess and evaluate your specific condition in order to provide rehabilitative and preventive procedure. This is coupled with short-term psychotherapy to help you psychologically adjust to living with HIV/AIDS. These services can also be extended to general homemaker services to ensure that you can live safely and independently.
